실습 21   Porcelain laminate veneer prep. (#12)

 
I. 실습 목적
Dentoform model에서 올바른 자세로 상악 측절치의 porcelain laminate veneer 지대치 형성을 실습한다.
 
II. 이론
A. 심미적인 수복 시술
1. cosmetic contouring - contour만 문제될 때
2. bleaching - color, stain의 문제
3. esthetic restoration - 치아의 특정 부위에만 심미적 문제가 국한될 때
4. crown
5. orthodontics
6. orthognatic surgery
7. esthetic bonding - composite resin or porcelain
B. Composite resin bonding의 장점
1. 한번의 약속
2. 치과의사가 형태와 색조를 직접 조절
3. 비용의 감소
4. 재수복이 가능
C. Porcelain laminate의 장점
1. ceramist의 예술적 기술을 활용하여 치과의사의 시간 단축
2. multiple unit를 짧은 시간에 장착 가능
3. porcelain의 색조 안정성, 심미성, 마모저항성, 조직과의 compatibility
D. Try-in and bonding
1) retraction cord
2) glycerin으로 trial seat check하고 excess proximal contact은 porcelain abrasive wheel로 제거
3) 필요하면 tint modifier(red, blue, orange, white, brown)와 uncured resin으로 시적하고 색조 평가
4) 아세톤으로 세척, 건조시킨 후 silane coupling agent apply
5) 지대치에 pumice, phosphoric acid etching, washing, dry
6) mylar strip으로 인접치아와 분리
7) bonding agent apply 후 resin cement apply. 상아질이 노출된 경우 그 부위에 상아질 접착제를 먼저 도포한다.
8) light를 차단하면서 finger pressure로 장착
9) polymerization
10) finishing, polishing with fine diamond and rubber point
E. Lingual chamfer로 incisal wrap할 경우
1) incisal thickness가 얇을 때
2) incisal lengthening이 1-2mm 필요할 때
3) facioincisal margin이 비심미적일 때
4) incisal부에 교합력을 받을 때

 
III. 실습 준비물

IV. 실습 과정
◇ POI이 facioincisal 방향이므로 incisogingival convexity는 유지됨
1. round bur shank를 guide로 cervical 1/3은 0.3mm, 나머지 부위는 0.5mm controlled depth reduction한다.
2. labial surface를 fissure bur로 균일하게 삭제하고 carborundum point로 마무리한다.
3. mesio-distal에 발생한 undercut를 제거하고,모든 margin은 ling chamfer로 finishing하면서 삭제된 표면을 매끄럽게 한다.
4. incisal edge는 1mm 삭제하고 절단의 설측에서 deep chamfer margin 형성

 
V. 주의 사항
. incisal edge를 넘어 설측으로 과도하게 넘어가지 않도록 할 것
. 실제 임상에서는 high speed handpiece와 porcelain laminate veneer system이라는 diamond point kit을 사용함.
