# Dental implant 란?
oral cavity에서 점막을 뚫고 tissue내로 삽입되어 보철물을 지지할 수 있는 device이며, post가 구강내로 노출되어 인공 abutment로 사용됨.
 
History of modern implantology

1937년(Venable); surgical Vitallium개발, Co-Cr-Mo alloy

Classification of dental implant
1. endosteal implant
bone을 뚫고 bone내부에 매식
1) blade vent implant
2) tooth root replacement
vitreous carbon, aluminum oxide, vitallium
3) osseointegration implant
titanium, hydroxyapatite
2. subperiosteal implant
soft tissue의 flap을 젖히고 bone implant을 뜬 다음 꽉 맞게 얹힐 수 있는 구조물을 주조하여 다시 삽입하는 방식.
1) mandibular subperiosteal implant
full arch인 경우 주조 post 4개가 밖으로 나와 C D를 지지한다.
2) maxillary pterygoid extension implant
상악의 bone은 흡수가 잘되므로 비교적 bone이 constant한
(1) hard palate
(2) ant. nasal spine
(3) pterygoid process
까지 연장하여 support를 얻음
3. mandibular staple bone implant
edentulous atrophic Mand.의 ant. portion에 장치하며 외과수술에 의하여 inferior Mn. border에서 부터 alveolar. ridge crest까지 구멍을 뚫고 staple을 끼워 고정한다.
4. endodontic implant
endo 치료한 치아가 동요가 심해서 지대치로 사용할 수 없을 때 치아장축 방향으로 root apex를 통과하는 기다란 post를 bone에까지 연장되게 박는다.
5. mucosal insert(button implant)
상악 CD의 retention 위하여 점막 내에 국한되는 조그마한 구멍을 뚫고 soft tissue의 undercut를 이용
6. ramus frame implant
Mand.의 anterior part와 ramus에 foot를 갖는 frame을 Mand.에 장착하고 이 위에 CD 해준다.
 
Biocompatibility of implant
1. biocompatible material
1) metal; titanium, Vitallium(Cr-Co-Mo), tantalum
2) carbon
3) ceramic & hydroxyapatite
2. consensus conference
1978년 Harvard univ.에서 보철과, 외과 의사 및 통계 분석가와 보사부 직원들이 모여 implant에 대한 clinical result를 조사 검토하였다. 1988년 NIH consensus conference 이후 치과계에 긍정적인 반응이 생김.
3. histologic evidence around implant body of blade vent implant
1) peri implant memb.(=pseudo periodontal memb.)
매식체와 주위골 사이에 생긴 결체조직 막이며 수직적으로 배열되어 있다.
2) implant body의 surf는 cementum의 surf. texture와 비슷하게 제조된다.
3) vent 내부로는 bone이 성장해 들어간 모습
4) 매식체 지대주 주위 조직은 정상치아 치은 열구 조직과 유사한 양상.
4. osseointegration
Sweden Brånemark의 연구결과로 2-stage surgery시 titanium 표면에 bone이 direct apposition됨이 광학 현미경 상으로 확인됨
 
Osseointegration cylindrical implant
1. 구조
fixture, cover screw, abutment, abutment screw, gold cylinder, gold screw
2. contraindication
1) irradiated patient
2) psychiatric problems
3) systemic disorder
4) pathology of bone and soft tissue
3. diagnosis and treatment plan
1) history
2) oral exam.
3) study cast
4) radiographic exam.
5) diagnostic wax up and splint 제작
6) bone evaluation
7) design; bone anchored vs. overdenture vs. splinting
3. appointments
1) examination, diagnosis
2) treatment planning
3) first surgery, fixture
4) suture removal - 1 week later
5) soft denture reline - 1 weeks later
6) relining of denture - every 3 weeks
상악; 4-6개월, 하악; 3-4개월
7) second surgery, abutment connection
8) suture removal - 1 week later
9) prosthesis - 2 weeks later

Blade vent implant
1. 구조
2. indication for endosteal blade vent implant
1) blade vent의 implant post는 removable PD(RPD)의 지대치로 사용되어서는 안된다.
2) single tooth의 replacement로는 사용해서는 안된다.
3) 상악동이나 inf. alv. canal을 피할 수 있을 만큼 충분한 bone이 있는 경우에만 사용한다.
4) 반드시 인접 자연치와 splinting하여 사용한다.
5) complete edentulous arch에서는 최소 4개의 implant에 의해 지지됨
3. procedure
1) selection of blade vent implant
X-ray와 clinical measurement에 따라 적당한 크기와 형태의 implant를 선택(sinus, nerve, canal 등 주의)
2) incision
implant 보다 5mm 길게 incision하고 mucoperiosteum의 flap을 재낀다.
3) channeling of bone
knife edge의 alv. bone ridge나 irregular한 area를 bone file로 다듬은 다음 길이가 긴 700XL, 700XXL bur를 이용하여 implant 보다 1mm 길게 groove 형성.
4) insert the implant
channel 내의 bone debris를 제거하고 깊이를 확인한 다음, titanium tip으로 된 seating instrument로 잡고 가볍게 malleting하여 제 위치로 장착시킨다.
(neck의 safety stop & lack 부위가 alveolar bone crest에 오고 shoulder는 2mm 하방에 위치)
5) suture
implant post부위는 semilunar punch로 excess한 soft tissue를 제거하고 interrupted suture with 3-0 black silk
6) temporary bridge
implant 의 stabilization과 환자의 psychological benefit를 얻기를 위해서이며 temporary cement가 incision line 내로 들어가지 않도록 rubber dam으로 protection
7) permanent prosthesis & routine check
