Terminology

1. prosthesis(補綴)

인체의 결손부를 인공적인 대치물로 대치시켜 주는 것. ex) 의족, 의안, 의이 등도 포함

2. prosthodontics(dental prosthesis. 齒科補綴學) 치의학의 한 분야로서. 치아의 치관을 수복하거나(crown. dowel crown) 결손치아와 이에 관련된 결손조직을 인공적인 장치로 보완해서 생리적인 기능이나 외모, 건강, 편안감을 회복시켜주는 과학이며 예술(art and science )

function(engineer) - 저작, 발음

esthetic(artist) - 특히 전치부

biological effect(doctor) - 인술

3. crown & bridge prosthodontics

단일 치아를 완전히 수복(crown)하거나, 혹은 고정성 인공장치물로 한 개 이상의 결손치아를 보충시켜주는 치과 보철학의 한 분과.

4. crown

1) 손상된 자연치를 수복하기 위해서 금속이나 도재, 혹은 resin으로 감싸줌으로써 자연치의 해부학적 형태를 회복시켜주는 수복물

2) 피개 정도에 의한 종류

(1) full veneer crown - crown 부위의 surface 들을 완전히 감싸서 수복

(2) partial veneer crown - 한 면은 감싸지 않고 수복(three quarter crown

3) 재료에 의한 종류

(1) metal crown

연성, 전성의 장점이 있는 gold를 많이 사용 - 주로 구치부 gold는 부식, 변색에 resistant 하고, 용융온도가 비교적 낮다 주조시 미세 부위까지 casting이 잘되고 toxicity 없을 뿐 아니라 esthetic상 장점을 갖는다.

(2) jacket crown - 주로 전치부에서 esthetic한 이유로 single crown으로 사용된다. resin J. crown과 porcelain J. crown이 있다.

(3) porcelain fused to metal crown, resin veneered metal crown.

- (1)과 (2)의 장점을 결합하여 심미적이며 강도 보강된 crown으로 bridge의 retainer로 사용할 수 있다. 구조는 metal로 framework 주조후, 표면에 porcelain 이나 resin으로 입힘.(lingual측은 metal)

cf) resin: 경화시 수축, metal과 붙지 않은 성질; loop나 bead를 만들어 기계적으로 부착시킴.

porcelain: 고온에서 metal과 서로 화학적 기계적으로 결합

5. bridge

non removable한 보철물로서, 한 개 이상의 결손 치아를 수복시키기 위해, 결손치에 인접한 치아를 지대치로 이용하여 부착시킨 고정성 보철물

6. removable bridge

적용: 치아 삭제를 싫어하거나, 나이 어린 patient

cf) removable partial denture인 경우는 치아뿐만 아니라 주위 조직도 이용(edentulous ridge)하고 치아나 의치상은 resin으로 구성됨.

removable bridge는 모든 교합압이 인접 지대치에 의해서만 support를 받고 의치상이 없다. 그러나 removable하고, clasp에 의해 유지되며 design은 bilateral로 되어있는 partial denture의 일종이다.

4 Components of bridge

1) abutment(支垈齒)

보철물을 유지해 주는 자연치아나 치근

2) retainer(維持 장치)

삭제된 지대치를 재형성시켜주는 restoration으로 bridge의 한 part(예; gold crown. inlay. dowel crown 등) 여기에 지대치가 부착되며 pontic이 연결된다.

3) pontic(架工齒)

결손치의 치관부를 심미적 기능적으로 대치시켜주는 인공물원래 자연치의 공간을 그대로 재현시키지는 않는다.

4) joint(connector, 連結 장치)

dental bridge의 한 part로 retainer와 pontic을 결합시켜 주거나, bridge pontic 들 사이의 각 연결 부위.

(1) rigid joint(solder joint)

(2) semirigid joint

(3) non - rigid joint(stress breaker): non - rigid joint는 당기면 빠짐

History

1. 보철의 시작은 fixed type.

- B. C 270. 페니키아의 유골 하악에서 gold wire로 splinting된 fixed type의 태초의 bridge 발견.(옛 Sidon의 무덤)

pontic: 상아 사용

joint: gold wire 사용

2. 문헌상. 12c 인도에서. denture 장치

3. 13c 마르코폴로 [동방견문록]

- 동남아. 젊은 남녀들이 gold foil로 치아를 씌워 장식 효과.

4. 16c 중엽. Spain

- 뼈, 상아를 깎아 partial denture로 사용

5. 17c 중엽. Japan

- 나무로 만든 removable partial denture 등장

6. 1728년. Pierre Fauchard

- " 치과의학의 아버지". 치과에 관련된 자료 수집하여 최초의 치과 서적 발행.

- 일반외과에서 치과 독립

7. 1792년 de Chamant

- ceramic(도재) 이용한 인공치 제작법 소개

8 1805년 Garriot

- TMJ의 hinge axis theory에 입각하여 교합기를(articulator) 발명

9. 1858년 Bonwill

- incisal edge와 좌우 TMJ 간의 거리가 4 inch 임을 주장. 4 inch triangle theory(교합기 제작시 참고됨)

10. 1878년 Richmond

- 치아의 coronal portion defect가 심할 때. root를 이용하여 crown을 제작하는 방법 제창.

11. 1906년 Carmichael

- Partial veneer gold crown의 초기 형태인 Carmichael's inlay제작.

12. 1907년 Tinker

- Carmichael's inlay를 개량하여 3/4 crown 제작(axial 측의 4면 중 한면은 intact한 crown)

13. 1907년 Taggart

- lost wax tech.의 casting method를 치과 보철에 최초로 도입.

- 현재도 lost wax tech.을 치과 주조법으로 사용.

14. 1950년대 이후, 재료학적, 기계 공학적, 교합학적인 많은 발전.

(1) rubber base imp. mat.

(2) high speed hand piece - ultra high speed air turbine handpiece

(3) PFM crown 등장

(4) gold alloy, ceramic, implant

(5) fully adjustable articulator.

Types of bridge retainer

1> extracoronal retainer(full veneer, partial veneer crown)

2> intracoronal retainer(inlay retainer)

3> intraradicular retainer(dowel crown)

1. extracoronal retainer

A. full veneer crown(complete V.Cr)

1) full veneer metal(gold) crown

2) jacket crown(complete resin, porcelain)

3) porcelain fused to metal crown

4) resin veneered metal crown

B. partial veneer crown - 모두 gold crown임

1) 3/4 crown

일반적으로 labial 측만 open되어 치질이 노출

2) modification

pinledge retainer

7/8 crown(buccal surf.의 mesial half 측만 open)

proximal half crown

reverse 3/4 crown

MacBoyle retainer

2. intracoronal retainer(= inlay retainer)

proximal 쪽을 포함하는 inlay로 치아의 내면으로부터(cavity) retention을 얻는다.

1) 지대치 상태 양호하고

2) 치열이 바른 경우

3) 교합이 정상적인 경우에 사용

4) missing space가 적은 short space에서만 사용

3. (intra) radicular retainer(dowel crown)

root 내로부터 retention을 얻는다. root에 post를 박아서 crown 회복

1) dowel retainer: Richmond Cr.부터 시작한 가장 오래된 것.

2) endo post: post에 맞는 drill까지 set로 기성 제품이 판매됨 - drill로 hole 형성 후 post를 끼워 넣는다.

3) Kurer system: 돌려서 나사식으로 삽입하는 post인 것이 endo post와의 차이

4) para-post system: endo post같은 것을 박고, 이것으로 retention이 부족할 때 더 작은 post를 1-2개 더 박는다.

5) custom made dowel core & crown: post와 core를 먼저 주조하여 제작하고 post를 cementation 하여 지대치 회복 후, impression 떠서 이 위에 정상적인 crown 제작하여 장착함.

Advantages of bridge

4. maintain the tooth position & supporting structure - 인접 dental arch의 position이 변하고 supporting structure가 변하는 것을 방지하고 건강 유지.

When a space is unfilled

1. 인접치아의 drifting, 대합치의 elongation

- loose contact point

- food impaction

- caries & periodontal diseases

2. occlusal interference

- mandible deviation(normal chewing cycle에서 이탈)

- bruxism

- TMJ pain dysfunction syndrome

3. esthetic, speech, chewing efficiency에 문제 발생

Cantilever bridge

1. definition

- bridge의 retainer가 pontic의 양끝 쪽에 존재하지 않고, 한쪽만의 abutment에 retainer를 만들어 pontic과 연결시킨 bridge.

한쪽에 인접치가 없을 때 다른 쪽에만 두개의 치아에 연결

2. indication(조건)

1) missing span의 space가 좁아야 한다.

2) abutment supporting structure가 건전해야 한다.

3) abutment root가 길고, 이상적으로 생겨야 한다.

4) pontic에 가해지는 교합력이 아주 경미할 때

3. 임상 적용 예

1) 하악 제 2대구치가 missing 시 대합치의 extrusion 방지를 목적으로 #35,#36 번을 이용하여 연결, 소구치 size의 작은 pontic을 #37에 사용

2) lst premolar of lower jaw의 missing 시, 바로 앞이나- 상악 치아와 (P1) 별 contact이 없다. (distal 만 약간) 1st molar를 crown 해야할 때 뒤의 치아만을 이용하여 수복한다.

3) 상악 측절치 missing 시, canine을 이용하여 수복 lingual rest seat & rest로 걸리게만 해준다.

Dental implant

1. definition

oral tissue 내에 삽입되어 crown이나 denture를 support하는 device이고, missing tooth를 대치하는 artificial abutment 이다

blade Vent implant -두께 1.2mm의 blade에 vent가 뚫려있고implant post와 이를 연결하는 neck로 구성됨

2. implant material: biocompatible material(foreign body reaction 을 일으키지 않은 물질)

metal - titanium, Cr-Co alloy(Vitallium, Tantalum)

ceramic, hydroxyapatite

carbon - vitreous carbon

3. dental implant의 불리한 점

구강내로 연결되어 세균 infection의 source가 될 우려

강력한 occlusal force가 가해짐.

o ideal occlusal surface. 회복 - ideal occlusion

o gingival sulcus에 infection없도록 periodontal health에 신경을 써야 한다.

Clinical & laboratory procedure for bridge

1. diagnosis and treatment planning

CR , CO check bite을 뜬다.

4. impression taking - working cast와 die를 제작키 위해.

5. working cast, die

6. mounting the cast on the articulator: 모델을 교합기에 mounting 시킨다. - 상. 하악의 jaw movement checking

7. wax pattern 제작: inlay wax 사용

8. sprue pin을 부착하고 investing

9. casting: crown & pontic 완성

10. soldering joint 형성

11. try in and adjustment

12. polishing
